Core Benefits -- Supplemental Benefits -- Part-Time Benefits

The following summary outlines the benefit options that we offer to all eligible full-time employees. Full-time employees are defined as employees working at least 32 hours per week.

Medical
Imagine One offers participants optional medical plans offered by Anthem Blue Cross and Shield. Coverages include well and sick visits; labs, x-rays and other types of tests; emergency and urgent care; maternity visits before and after having a baby; hospitalization; vision and Employee Assistance Program. Plan highlights are as follows:

    Anthem KeyCare - Plans that offer both in and out-of-network benefits. The doctors, hospitals and other health care professionals in the Anthem KeyCare network agree to accept a set fee as payment for covered services, thereby helping to contain medical costs. Anthem KeyCare features a two-tiered benefit design which allows members the option of seeing any provider. They pay less when using a Preferred Provider Organization (PPO) network provider, but have the flexibility to use an out-of-network health care provider while shouldering a greater share of the cost.

    Anthem Blue View Vision - Another great Anthem KeyCare feature! Plan covers a variety of vision services and benefits, value-added savings, and access to a broad network of providers.

    Dependents eligible to participate on plan through end of year after reaching age 23 (non-student or full-time student).

    Imagine One pays 90% of employee coverage and 20% of dependent coverage.

    Coverage (should you elect this benefit) begins on the 1st day of the month after your start date.

Dental
Imagine One participates in Delta Dental Premier dental program. Under this program you may use any dentist you wish; however, out of pocket costs may be lower when you select a participating dentist. Participating dentists agree to adhere to Delta Dental processing policies and are prohibited from billing a patient above the pre-negotiated fee, accepting billing under these terms as payment in full resulting in savings to our group plan. The Delta Dental Premier program has the nation's largest dental network, with more than three out of every four of the country's dentists participating. The Delta Dental Premier provides two levels of protection that is unmatched.

Benefits from the Delta Dental Premier include:

    100% reimbursement of covered expenses for Diagnostic and Preventive Care (oral exams, cleaning, flouride applications, bitewing x-rays; space maintainers; sealants, etc.). These services are exempt from the deductible.

    80% reimbursement of covered expenses for Basic Dental Care (fillings; stainless steel crowns; oral surgery; denture repair and recementation of crowns, bridges and dentures; root canal therapy, periodontic services, etc.). Deductible applies.

    50% reimbursement of covered expenses for Major Dental Care (prosthodontics/dentures/bridges/crowns). Delta Dental Premier will pay the stated percentage of the plan allowance based on the dentist’s participation.

    Coverage (should you elect this benefit) begins on the 1st day of the month after your hire date. Coverage shall end on the last day of month after your termination.

Participating Dentist

    Payment will be made directly to the dentist. Delta Dental Premier’s payment will be based on the allowance for covered benefits.

    The dentist will accept Delta Dental Premier’s payment, plus any required co-insurance and deductible (if applicable) as payment in full for covered benefits.

Non-Participating Dentist

    Delta Dental Premier’s payment will be based on the non-participating dentist allowance for covered benefits.

    You will be responsible for any required co-insurance and deductible (if applicable) as well as the difference between the non-participating dentist’s charge and Delta Dental Premier’s payment for covered benefit. The amount you would owe a non-participating dentist may be higher than the amount you would owe a Delta Dental Premier Dentist for the same covered benefits.

Medical Spending Account
Pre-tax dollars can be set aside to pay for out-of-pocket medical expenses.

Dependent Care Spending Account
Pre-tax dollars can be set aside to pay for approved day care expenses.

Employee Assistance Program (Core Benefit)
Anthem Employee Assistance Program (EAP) - Anthem EAP offers employees and eligible dependents three sessions per crisis with licensed behavioral health counselors, at no charge. Crisis services include, but not limited to, mental health, alcohol/drug abuse assessment, child or elder care, grief counseling and legal/financial services.

Short-Term & Long-Term Disability Coverage (Core Benefit)

    Under our Short-Term Disability policy you are entitled to a benefit equal to 60% of your weekly earnings up to a maximum of $500 per week. This amount is payable to you should you become temporarily disabled for a period of at least 31 days. The cost of this benefit is paid for by Imagine One.

    Our Long-Term Disability policy covers you if you lose part of your income or are unable to work due to a total disability. Your benefit will begin after you have been disabled for a period of at least 180 days. If you qualify you will receive 60% of your monthly earnings up to a maximum of $5,000 per month. The cost of this benefit is also paid by Imagine One.

    Coverage begins on the 1st day of the month after your start date.

Life Insurance (Core Benefit)

    Our Life and Accidental Death & Dismemberment insurance is also provided through Prudential Life Insurance Company. The total cost of this benefit is completely paid for by Imagine One.

    You are automatically covered for life insurance if you are a full-time employee working at least 32 hours per week.

    Coverage is equal to your annual salary up to a maximum of $100,000. Your salary base is determined for the year based on your salary on December 31st of the previous year, or on your new hire salary if you are hired in the current coverage year.

    The total amount of your death benefit is automatically reduced by 35% when you reach age 65.

    The benefit is reduced to 50% of the original amount when you reach age 70.

    Coverage begins on the 1st day of the month after your start date.

Maternity and Paternity Benefit (Core Benefit)

    For employees who take leave for maternity or paternity, one week of your salary will be paid during the first week of maternity or paternity leave. Vacation leave or leave without pay must be taken after that until the STD benefit begins.

    Coverage begins on the 1st day of the month after your start date.
