The following summary outlines the benefit options that we offer to all eligible part-time employees. Part-time employees who are eligible for benefits are defined as an employee working at more than 24 hours but less than 32 hours per week. These benefits do not cover any Imagine One Wage Determination Employees.

Medical
Imagine One offers participants optional medical plans offered by Anthem Blue Cross and Blue Shield. Coverage includes well and sick visits; labs, x-rays; and other types of tests; emergency and urgent care; maternity visits before and after having a baby; hospitalization; vision; and Employee Assistance Program. Plan highlights are as follows:

    Anthem KeyCare - Plans that offer both in- and out-of-network benefits. The doctors, hospitals, and other health care professionals in the Anthem KeyCare network agree to accept a set fee as payment for covered services, thereby helping to contain medical costs. Anthem KeyCare features a two-tiered benefit design which allows members the option of seeing any provider. They pay less when using a Preferred Provider Organization (PPO) network provider, but have the flexibility to use an out-of-network health care provider while shouldering a greater share of the cost.

    Anthem Blue View Vision - Another great Anthem KeyCare feature! Plan covers a variety of vision services and benefits and provides value-added savings and access to a broad network of providers.

    Imagine One pays 90% of employee coverage. Coverage is only offered for the individual employee.

    Coverage (should you elect this benefit) begins on the 1st day of the month after your start date.

Employee Assistance Program (Core Benefit)
Anthem Employee Assistance Program (EAP) - Anthem EAP offers employees and eligible dependents three sessions per crisis with licensed behavioral health counselors, at no charge. Crisis services include, but are not limited to, mental health, alcohol/drug abuse assessment, child or elder care, grief counseling, and legal/financial services.

AFLAC Insurance

All employees are eligible to participate in the AFLAC program. AFLAC, a Fortune 500 company established in 1980, offers supplemental accident/disability, short-term disability, hospital intensive care, hospital confinement indemnity, long-term care, specified health event, life, and much more. Employees will contact AFLAC agent directly for additional information on plan benefits and enrollment. Most of these items are pre-tax and are subject to the pre-tax requirements for enrollment. All AFLAC insurance is paid 100% by the employee.

Pre-Paid Legal Plan

All employees are eligible to participate in this program. Employee and family can access legal counsel and advice from a qualified network of attorneys and get unlimited legal consultation and services for all their legal needs -- phone calls and letters, contract and document review, will preparation, and expanded trial defense benefits are just a few. This plan offers protection, security, and peace of mind and is paid 100% by employee.

Identity Theft Shield

All employees are eligible to participate in this program marketed by Pre-Paid Legal Services, Inc. Identity Theft Shield will provide you with peace of mind -- providing early detection with continuous credit monitoring and Identity Restoration Services. This plan, backed by experienced professionals from a division of the world's leading risk consulting company, covers you and your spouse. This plan is paid 100% by employee.

Benefit Enrollment Process

What do you need to do to enroll?
Complete and sign the appropriate enrollment forms and return them to Human Resources. You have 15 days from the date you are eligible for coverage to submit your completed enrollment forms to the insurance carriers. If you miss this due date, you will have to wait until our next Open Enrollment Period to elect coverage. Open enrollment is held in the 4th quarter of the year for changes for the following calendar year.

Note: After your initial enrollment period, you cannot make changes to your coverage during the year unless you experience a change in family status, such as:

  • Loss or gain of coverage through your spouse.
  • Loss of eligibility of a covered dependent.
  • Death of your covered spouse or child.
  • Birth or adoption of a child.
  • Marriage, divorce, or legal separation.
  • Switch from part-time to full-time.

You have 30 days from a change in family status to make changes to your current coverage.

Tuition Reimbursement

You are eligible for tuition reimbursement up to $1200.00 per year. Tuition reimbursement is paid when you enroll in the class. It must be repaid if you fail the class or if you leave the company before one year from the completion of the class. See the tuition reimbursement policy for details.

Paid Time Off (Including Sick, Vacation & Personal Leave)

Paid time off is prorated for part time employees working less than 32 hours per week, but more than 24. It is based on 10 days per year leave time. No more than 80 hours can be carried over a calendar year.

New Business Bonus

A Bonus of $1500 will be paid for any significant new contract or task work marketed and secured by an employee. $1,000 of the bonus will be paid on the pay date after the signing of the contract. The rest of the bonus will be paid six months after the new work commences. If the work is terminated (without being completed) before the six-month bonus payout date, there will be no final payout. The employee must be employed with Imagine One at the time of the bonus payout in order to receive the bonus. Managers should email HR for approval.

Recruitment Bonus

A bonus of $750.00 will be paid for recruitment and hire of a full-time applicant for greater than $30K salary. A bonus of $350.00 will be paid for recruitment and hire of a full-time applicant for less than $30K salary. $100.00 of the bonus will be paid on the pay date after the employee has been begun working for Imagine One. The remaining balance will be paid after the referred employee has completed 90 days of employment with Imagine One. Both the recruiting employee and the recruited employee must be employed with Imagine One at the time of the bonus payout, or no bonus is paid.

Part time employees working more than 24 hours per week are entitled to prorated holidays.

Termination of Benefits

Medical coverage ends on the last day of termination month.
Vacation balances will be paid out on the final paycheck or on the next pay cycle, depending on the amount of notice.
